What information should be on the Transport Document?
The differences between the modes is the need for additional information
relating to the transport operation rather than the nature of the goods. The
following dangerous goods information must be shown on the transport document:

the UN number preceded by the letters "UN"

the proper shipping name supplemented, when applicable with the technical
name in brackets.

the Class number and any additional class hazards.

the packing group, if assigned, preceded by the letters PG.

the tunnel code (if applicable)

the number and description of the packages.
-
the total quantity of each item of dangerous goods.
-
the name and address of the consignor (sender) of the goods.
-
the name and address of the consignees (receivers) of the goods.
-
a declaration as required by any special agreement.
Modal and Multimodal dangerous goods forms have specific boxes which must
be completed with the additional information regarding the transport operation.
(e.g Name of vessel, Port / Airport of loading / destination etc..) Additional
information such as flashpoint for Class 3 flammable liquids is also required
for sea freight.
